What insurances are accepted?
We accept all major commercial insurance PPO plans and Medicare Part B. If you are not sure if your plan is covered – we suggest calling the number located on the back of your insurance card under member services. A representative will go over your specific coverage plan.
Do you accept Medi-Cal/Covered CA?
Unfortunately, our private office does not accept Medi-Cal insurance. We are happy to still see patients who have Medi-Cal as a concierge service with private pay pricing.
What are the private pay prices?
We offer “Good Faith” prices to our private cash pay patients; meaning that we charge the price that Medicare Current Year lists for the specific billing codes.

I want to be a new patient, what next?
Determine if you need a referral (Non-HMO plans like Medicare and PPO do not require referrals). Not all patients require a referral. In general, HMO plans or managed Medicare do require referrals from a primary care physician. Once we receive a referral, we will call you to setup an appointment and review details.
If you do not hear from us and would like to schedule immediately, please do not hesitate to call us. We have schedulers standing by.
We only schedule patients who have a complete referrals received by us. Referrals can be sent by email, fax, or uploaded to the website portal.
A complete referral includes:
● Patient Referral form from referring Physician with diagnosis
● Patient Medical Records (recent progress notes, imaging, labs, and any hospitalization records)
● Medical Authorization (depending on your insurance)
● Patient Demographics; including insurance information
What is your appointment cancellation policy?
Patients are given courtesy appointment reminders 2 days prior to their scheduled appointment via phone,SMS text, or email. The patient is obligated to adhere to ARSM appointment policy which includes:
● If a patient does not show for an appointment, the No Show Fee of $50 will be invoiced by SKAND Corporation.
● If there is no confirmation from the patient, the appointment may cancelled and the patient may be subject to the No Show Fee of $50.
● Cancellation policy is 24-hours in advance of the patient’s scheduled appointment, otherwise fees may apply.
Cancellations can be done by Phone or Email .
Do you accept workman's compensation claims?
Unfortunately, our office does not provide worker’s compensation care claims.
What payments methods are accepted?
We accept all major credit cards and checks made payable to SKAND Corporation.

Please note that our billing is not done in-house, we use a third-party company for all billing inquiries. Any invoices sent to patients will be from SKAND Corp, A California Medical Professional Corporation.
